Tdap Clinic

Great News!

The Caribou School Department Health Services Office will be offering a Tetanus, Diphtheria, Pertussis (Tdap) Clinic in February.

The clinic will be free to all children 11-18 years of age in the Caribou School System.  Caribou Middle School students will be transported to the High School by bus.

Students needing a Tdap Immunization will be receiving their paperwork home with their child’s progress report.  Paperwork will need to be returned by January 23, 2009.

If your child doesn’t need the immunization because he/she received it at your physician’s office,

please bring in the shot record to be copied or have your provider’s office fax it to us at 493-4254.  State law requires that we view the actual written immunization record as provided by your health care provider.  (We cannot take this information over the telephone).

You may contact the school nurses office at 493-4255 with any questions.
